Planescape Monsters for 5e - The Visage

If you're running Dead Gods, there's no statblock more important. For all of AD&D's sprawl of rules, the reality manipulation is left wildly open to the DM. It's doesn't even in one consistent way through the adventure!

Whatever you do, be aware that if you use the reality manipulation "well", your combats will be much longer than they normally are. Be aware also that visages tend to add a lot of confusion, but little in the way of tension. In a fight against so many illusions, your players might be curious and compelled; but they may also end up bored and frustrated. Especially low-Wisdom PCs.

Innate Spellcasting. The visage's innate spellcasting ability is Charisma (spell save DC 14). The visage can innately cast the following spells, requiring no material components:

1/day: dominate person

Magic Resistance. The visage has advantage on saving throws against spells and other magical effects.

Turn Resistance. The visage has advantage on saving throws against any effect that turns undead.

Actions

Multiattack. The visage makes two attacks with its claws.

Claws. Melee Weapon Attack: +7 to hit, reach 5ft., one target. Hit: 9 (2d6 + 3) slashing damage.

Lucidity Control. The visage taps into the mind of up to six creatures that it can see, taking total control over a target's senses. This ability has no effect on undead or constructs. A creature that suspects their senses are being manipulated can make a DC 15 Wisdom saving throw at the start of their turn, ending the effect on a success. This effect lasts as long as the visage maintains concentration.

Steal Essence. The visage steals the essence of a humanoid within 10 feet that has been dead for no longer than 1 hour. The visage assumes the form of the target, including any nonmagical equipment being worn by the creature. The visage learns any proficiencies the creature had in life, as well as its general habits and mannerisms. While in this form, the visage cannot use its Claw attack. If the visage maintains the humanoid's form for 24 hours, or if it discards the form, the creature's soul is destroyed and no mortal magic can return them to life.
